AI watches the brain: Cleveland Clinic and Piramidal bring foundation model to the ICU

The Cleveland Clinic and startup Piramidal are developing an AI system that reads continuous brain wave data from ICU patients to detect neurological changes in real time. This approach could transform critical care by shifting from periodic human review to full time machine assisted monitoring.

In today’s ICUs, patients with brain injury or seizure risk are tethered to EEG equipment that records their electrical brain activity via electrodes on the scalp. Physicians review these EEG signals after intervals, often every 12 to 24 hours, to spot signs of trouble. But during those gaps, patients might deteriorate unnoticed. Manually reviewing a day’s worth of EEG for one patient can take two to four hours.

The AI model being built by Piramidal is designed to continuously analyze the EEG stream and instantly flag abnormalities as they arise. Rather than waiting for human review or retrospective charting, the system would act as a real time sentinel, alerting clinicians to seizures, trends toward brain dysfunction, or shifts in consciousness. In effect, the model becomes a constant assistant watching over the brain.

Founders of Piramidal include Dimitris Fotis Sakellariou, a neuroengineer and AI researcher, and Kris Pahuja, a product strategist with prior experience at major tech firms. The startup was launched in 2023, raised seed funding, and is now collaborating closely with Cleveland Clinic to access de identified ICU EEG data. Their training dataset reportedly includes nearly one million hours of EEG from thousands of individuals in both healthy and diseased states. This breadth is essential, because brain wave patterns vary wildly across people, and the model must generalize across that variation.

As part of the collaboration, researchers are working with retrospective EEG records to develop and refine the algorithms. In the next six to eight months, they plan to initiate trials with live ICU patients under controlled conditions. During initial deployment, the system will be limited to a few beds rather than full wards to carefully manage false positives and false negatives. Misidentifying brain events is not just inconvenient, in critical care it could be dangerous.

If the system proves reliable, it might eventually scale to oversee many ICU beds simultaneously, reducing the labor burden on neurologists and critical care teams. The goal is not to replace clinicians but to augment them, equipping staff with a second set of “eyes” on brain health. In future, Piramidal hopes to expand into continuous monitoring for conditions like epilepsy or sleep disorders using similar brain foundation models.

Ethical, privacy, and regulatory questions loom large. How should brain data be stored, used, and protected? Who controls the alerts and decisions the AI makes? How do you audit errors? Experts argue that technologists, ethicists, clinicians, and patients must help shape guardrails now, before thousands of lives depend on them.

This brain reading AI model marks a major step toward embedding predictive intelligence into life saving care. In American hospitals, it could dramatically shorten reaction times to neurological emergencies, free up time for specialists, and improve patient outcomes. Yet success depends not only on technical performance but on establishing trust, rigorous oversight, and ethical guardrails. As foundation models expand beyond language into life’s most delicate systems, the stakes have never been higher.
